Community Health Needs Assessment Toolkit Part II: Completing the CHNA Report

This toolkit outlines decision-making criteria and process guidelines for the identification and prioritization of health needs, preparing KFH hospital facilities to complete their CHNA reports. After completing the identification and prioritization process and completing the CHNA Report, hospitals will be ready to select the health needs they will address through Implementation Strategies.
