Woven With Elders: Pacific Islander Vaccine Toolkit – New!

This vaccination toolkit was created for and by Pacific Islanders. “Woven with Elders” is specifically for community organizations that serve our Pacific Islander communities that have been devastated by COVID-19 with high rates of infection, hospitalization, and death. We encourage you to use and share our toolkit components that will be translated into various Micronesian, Polynesian, and Melanesian languages.

Pacific Islanders continue to face health disparities. To address this need, PI-CoPCE has identified “Four Pillars” or strategic focus areas to improve the health status and access of Pacific Islanders in the United States, Hawaii and U.S. Pacific: Primary Health Care Support; Data, Research and Evaluation; Workforce Development and Support; and Community-Based Organizations Training and Technical Assistance.

Pillar I: Primary Health Care Support

Improving access to linguistically and culturally appropriate primary health care services and programs for PI communities.

Pillar II: Data, Research and Evaluation

Increasing PI-specific health data, program evaluation, and research.

Pillar III: Workforce Development and Support

Creating, sustaining, and retaining a primary care workforce that will meet the needs of PI patients in the United States and U.S. Pacific.

Pillar IV: Community Organizations Training

Assisting PI-serving community-based organizations in their delivery of primary care services in support to community health centers.
