Community Health Needs Assessment Toolkit Part I: Overview, Pre-Assessment, & Data Collection

This toolkit was developed by Kaiser Permanente Community Health staff to support the implementation of Community Health Needs Assessments (CHNAs) and to ensure compliance with the Federal CHNA requirements and guidelines. CHNAS are used to understand the needs and resources within our communities and to guide our investment strategy.
